Description

Aging in place is a goal for many seniors who wish to stay in the community where they’ve worked, worshipped, raised their children, maintained their home, and their friendships. PACE Southeast Michigan, part of the national PACE (Program of All-Inclusive Care for the Elderly) program, is a unique health plan that provides home and community-based services to help chronically ill, nursing-home eligible seniors maintain their independence and remain in the community they love.

PACE Southeast Michigan brings together all the care needed – medical, social, behavioral, nutritional, rehabilitative, transportation services, and many others – in a truly integrated care model.

Come and learn about a program that offers a look at where the future of health care is headed. We will tour the center for an up-close look at the services offered by this innovative organization.

Street Parking is available: Attendees can also park for $5.00 in the Stroh parking lot. They enter off of Wight Street between McDougall and Joseph Campau. It’s about a half-block walk to PACE Southeast Michigan from there.
