What are the social determinants of health (SDOH)?

This presentation will introduce participants to the social influencers that are responsible for up to 80% of a person’s overall health and wellness. Illustrations of the concepts will include compelling patient stories and examples from metro-Detroit’s own history with food insecurity, redlining policies that restricted housing for minority populations, and regional transit policies. The presentation will end with examples of how Henry Ford Health System is attempting to overcome social health vulnerabilities for their patients.

Rev. Alexander Plum, MPH, MBA(c), CHES, is Director of Clinical and Social Health Integration within Primary Health and a Senior Investigator at the Global Health Initiative. This is his 7^th year with Henry Ford Health System. An ordained deacon in the United Methodist Church, his work transforms the institutions and infrastructure of the US health care system in order to achieve health and hope for vulnerable and marginalized communities around the world. He is internationally recognized in the field of reverse innovation and global learning in health care. Alex is a returned Peace Corps Volunteer, a former Paul D Coverdell Fellow, and Salzburg Global Fellow – but most of all, he’s a proud alumnus of Michigan State University. Go Green!
